Input Parameters

Hypertension (uncontrolled, >160 mmHg systolic)*

Abnormal renal function (dialysis, transplant, Cr >2.26 mg/dL)*

Abnormal liver function (cirrhosis or bilirubin >2x normal or AST/ALT/AP >3x normal)*

Stroke history*

Prior major bleeding or predisposition to bleeding*

Labile INR (unstable/high INRs, time in therapeutic range <60%)*

Age >65 years*

Medication usage predisposing to bleeding (antiplatelet agents, NSAIDs)*

Alcohol use (≥8 drinks/week)*
