Critical Threshold

qSOFA ≥2: High risk for sepsis. Proceed immediately to full SOFA assessment and initiate sepsis bundle.

Clinical Guidance

•Assess at bedside within 5 minutes of suspicion
•qSOFA ≥2 has 70% specificity for sepsis
•Even qSOFA 0‑1 does not rule out sepsis — use clinical judgment
•Obtain blood cultures BEFORE antibiotics if possible
