qSOFA ≥2: High risk for sepsis. Proceed immediately to full SOFA assessment and initiate sepsis bundle.

  • •Even qSOFA 0‑1 does not rule out sepsis — use clinical judgment
  • •Obtain blood cultures BEFORE antibiotics if possible
